This site only concerns the format of the message you received, not with the operator or policies that caused them to use the format.

Format information only specifies where the information goes in the report.

Again: This site has nothing to do with why the report was generated, who generated it, or what you can do to fix the problem.

If you decide to call the Mipassoc contact, Dave Crocker, all he will tell you is the same thing you have just read here.

Dave cannot help you with the problem cited in the notice you received. Really.
